Phase 1 tests safety and dosing in small groups (20-100 people). Phase 2 tests efficacy and side effects in larger groups (100-300). Both carry higher uncertainty but offer access to novel treatments.
Phase 3 compares new treatments to standard of care in large groups (300-3,000+). Phase 4 monitors long-term safety after approval. Phase 3 provides the strongest evidence.
Earlier phases offer novel treatments but with more uncertainty about benefit. Phase 3 trials provide the most reliable evidence and are closer to changing standard practice.
